Many serious accidents have occurred due to a machine started up when it was supposed to be shut off to prevent repairs or maintenance. These accidents are usually caused by stored energy that is not properly released. Lock-out tag-out (LOTO) systems help prevent these incidents by ensuring that machines remain completely off until an authorized person releases them.
The LOTO system is comprised of a variety of locks, tags and other devices used to secure, isolate or shut off a machine from an electric source. All "affected" and "authorized" personnel should be trained on the use of these devices. These devices include, but are not limited, to chains, locks and padlocks. Each device is uniquely identified and labeled, with the date and the time of application and the name of the authorized individual, and the reason for the lockout or tag-out (repair or maintenance, for example). It should be clear that these devices are not able to be removed until a full LOTO procedure has been carried out.
